Pinehaven House will support youth to meet and to assist the challenges of adolescence. Pinehaven House will assist youth in gaining the life skills and education they need to become self-sufficient, successful adults.
Pinehaven House is a corporate, staffed foster home for up to 8 adolescent males ranging in age from 12 up to age 20. Each adolescent living in the home must be willing to work on transition goals that will lead to independence or another approved goal. Youth residing at Pinehaven House may be offered the opportunity to reside in foster care up to the age of 21.
Program components include:
- Treatment planning and goal setting
- Development of interpersonal social skills
- Gaining time management skills
- Daily living skills/independent living skills.
- Work readiness and employment opportunities
- Educational interface and planning assistance
- Nutrition and meal planning
- Opportunities for practical preparation for transition
- Health/mental health/chemical health coordination of care
- Community volunteer projects
- Recreational and leisure activities
- House group to address various needs
- Weekly in-house independent living skills groups
Pinehaven Youth and Family Services, Inc. will provide case management services to residents. They will work directly with the placing agents and the resident’s placement team. They will monitor progress on goals identified in the resident’s Individual Program Plan.
Goal areas of the Individual Program Plan will include the following areas:
- Adjustment to community
- Family
- Interpersonal skills
- Education/Employment
- Health/Mental Health/Chemical Health
In addition, upon successful discharge, each youth will have created a Transition Portfolio. Included within this portfolio will be the following items:
- Copy of social security card
- Certified copy of birth certificate
- Driver’s License or MN State Identification card
- Medical and dental records
- Contact information for medical doctor, psychiatrist, dentist, and therapist
- Copy of bank information
- Past and present employment information
- Health insurance information
- Contact information for youth’s siblings, if applicable and in foster care
- Any other information pertinent to independent living and existing case plan
House Programming:
Pinehaven House will implement the following house programming:
- Weekly house meeting with all youth and any available staff members to discuss any home issues, create individual schedules for the coming week, upcoming events, changes to policies, or other relevant issues
- Independent living skills groups on a variety of topics
- Participation in daily chores, all household activities, and any other life skill building activities
